Payment & Fees

Life Choices provides treatment services on a private pay basis. Most insurance plans also are accepted. With the private pay method, services and information are not relayed to insurance companies but are kept strictly confidential. For clients that use insurance benefits, HIPAA (to include diagnoses) information is relayed to the insurance carrier. Regardless of how services are billed, all payments are accepted via check, cash, or debit/credit card. For card transactions, an additional processing fee is applied.


Other services apart from psychotherapy (including but not limited to: letters written by the therapist, standard start, attendance or completion forms, and evaluations) may have additional fees (Life Choices Fee Schedule under Forms).  Payment for these services is either handled in advance or at the time that service is rendered.

Since there are only a limited number of counseling hours available during the week, charges are assessed for missed sessions unless cancellation is made 24 hours in advance. As your insurance company will not subsume the cost for missed appointment charges, this fee is passed to you, the client.
